Traumatic brain injury can cause various pathological conditions both in the eyeball and cranial cavity. The presented clinical case demonstrates features of differential diagnosis direct carotid-cavernous fistula secondary post-traumatic glaucoma. Direct is formation a communication between internal carotid artery cavernous sinus discharge arterial blood into it. first symptom pulsating noise head above eye. manifest signs picture are obstruction venous outflow from eye orbit: proptosis, eyelid edema chemosis, congestive redness eye, varicose subcutaneous veins eyelids, as well dilated ophthalmic vein visualized by ultrasound orbit, hypertension, oculomotor disorders, state fundus. patients with contusion glaucoma, addition to an increase intraocular pressure, characterized presence changes cornea mild superficial opacities more dense ones, luxation or subluxation lens anterior chamber vitreous, destruction vitreous hemorrhage. similarity manifestations traumatic glaucoma difficulties diagnosis.

We report a case of post-traumatic, direct high flow carotid cavernous fistula that resolved spontaneously 06 weeks after carotid angiography.
